Diabetes Self-Management Classes


Diabetes Self Management Classes are offered in a supportive environment that can provide the tools and inspiration you are seeking in learning to live with diabetes. Our goal is to offer this service to you as a new patient in our practice during your first three visits to allow you an early start in reaching control.

A follow-up annual class is offered to keep you abreast of new concepts, medications, meters or to make sure you are on track with your Diabetes Health Checklist.

We encourage our patients as well as family members to attend classes. We are dedicated to being your partner in managing your diabetes successfully.

First Office Visit - Basics of Diabetes & Nutrition

Am I Type 1 or 2?
When should I monitor; how often?
Goals/Targets; What's my A1c?
Basic Nutrition; foods to eat
Pill, Insulin, what else is there?

Second Office Visit - Nutrition and Activity


Individual Consultation

Meal planning for me
Grocery shopping tips
How to count carbohydrates and why
Behavior changes I can make
How activity affects my glucose levels

Third Office Visit - Beyond Basics

What to do when I get sick
Complications I can avoid
Traveling with medications
Surviving the Holidays
New treatments; now and future
Goals review

Annual Review

As Needed - Insulin Pump Therapy
