What to Expect on your First Visit
We are pleased that you have chosen our practice to provide you with medical care. Your first visit to our office is very important to put your treatment plan in motion.
We are an ADA Certified Treatment Center and follow their guidelines to provide the highest quality care. If you have visited other diabetes treatment offices, you may notice some differences with regard to the amount of information we collect and the procedures we follow.
On your first visit you can expect the following:
- Laboratory work will be drawn when you first arrive
- Breakfast snack will be provided
- You will take your medications
- Meet with physician
- Meet with a Diabetes Nurse Educator (DNE)
- Meet with a Nutritionist
Your first appointment will last until approximately 12:00 Noon. Follow-up visits will be considerably shorter and are scheduled during your first visit.
First Visit Requirements
We request that you follow these guidelines before your appointment:
- Do NOT eat or drink anything (except water) for 12 hours prior to your appointment (if your blood sugar drops, drink juice or eat a glucose tablet)
- On the morning of your appointment, do NOT take any of your usual medications
- Bring ALL of your current medications (and medical equipment) with you to your appointment
- Bring your Insurance Card, Co-pay or Deductibles.
- If you have had any laboratory tests completed in the last 3 months, please bring a copy of the lab results (referring physicians can fax results)
- Fill out Medical History Forms and Patient Information forms listed below
