Insurance Coverage
We have participation agreements with the following insurance carriers:
- Aetna
- Blue Cross/Blue Shield
- Blue Grass Family Health
- CHA
- Cigna
- Medicare
- United Health Care among others
Our business staff will file insurance claims with all of your insurance claims with all of your insurance carriers and bill you for any un-reimbursed balance. Please plan to pay insurance deductibles and coinsurance at the time of your visit, as well as any balance due on your account.
It is important to bring all of your insurance cards with you on your first visit to the office. We also need to know which is primary if you have more than one health insurance policy. Please notify us immediately of any changes in your insurance carrier, address or coverage. Your insurance is a contracted agreement between you and your insurance carrier. Any statement regarding "usual and customary charges" is their opinion, not ours.
Patients without health insurance should be prepared to pay in full at
the time of service. You may make monthly payments if you arrange for a
plan, in advance, agreeable to us. Please contact our Billing and
Insurance Coordinator before your first visit.
Ultimately, payment of your bill is your responsibility. We accept personal checks, Visa and Master Card for your convenience.
